Panspinal Epidural Abscess
نویسنده
چکیده
A 41-year-old man with recent intravenous drug use presented with 2 weeks of progressively worsening neck and low back pain, four days of bilateral upper and lower extremity weakness, and urinary retention. Magnetic resonance imaging (MRI) revealed a T2/STIR hyperintense posterior epidural collection, suggestive of a spinal epidural abscess (SEA), extending throughout the epidural space of the cervical, thoracic, and lumbar spine (Figure 1). Axial T2-weighted imaging revealed compression of the cord with associated intramedullary T2 hyperintensity (Figure 2A). Large fluid collections within the right paraspinal muscles (Figure 2B) and bilateral psoas muscles are consistent with additional soft tissue abscesses.

Panspinal Epidural and Psoas Abscess with Secondary Cervical Disc Space Infection
Psoas9 and epidural spinal abscesses2, 6 have been described as relatively rare conditions. The incidence of epidural spinal abscess, however, has doubled in the recent two decades, reaching 1 in 10000 hospital admissions8. Early diagnosis and management of these two pathologies, especially when presenting with insidious and vague symptoms, require a high degree of suspicion. In this case, unus...
متن کاملA case of panspinal epidural abscess that presented with meningeal irritation
Case In rare cases, spinal epidural abscess involves the entire spine and can lead to neurological deficits and sepsis if treatment is delayed or suboptimal. A 65-year-old man was admitted with a diagnosis of bacterial meningitis. After admission, magnetic resonance imaging showed a spinal epidural abscess from the cervical to lumbar spine. Blood culture revealed Staphylococcus aureus. The pati...
متن کاملExtensive spinal epidural abscess.
Spinal epidural abscess (SEA) is a rare clinical entity associated with a high morbidity and mortality. It usually spans 3-4 levels. The literature contains very few reports of extensive or panspinal epidural abscesses and hence there is no consensus on management of such cases. We describe a case of 56 years old male presenting with a posteriorly located SEA extending from the Foramen Magnum t...

Despite the increasing prevalence of spinal infections, the subcategory of holospinal epidural abscesses (HEAs) is extremely infrequent and requires unique management. Panspinal imaging (preferably MRI), modern aggressive antibiotic therapy, and prompt surgical intervention remain the standard of care for all spinal axis infections including HEAs; however, the surgical decision making on timing...

To cite: Verma R, Chaudhari TS, Lachuriya G. BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/ bcr-2014-204892 DESCRIPTION An extensive spinal epidural abscess (SEA), sometimes referred to as a panspinal or holocord SEA, is a rare neurosurgical emergency. We present an interesting case of spontaneous extensive SEA that was managed with multilevel laminectomy along with...
